พอดีว่าเจอปัญหาเรื่อง DIs ระหว่าง warfarin กับ NSAIDs,omeprazole,cipro ยาดังนี้ครับ piroxicam 1*2 30 tab,omeprazole 1*1 ac 15 tab,cipro 250 mg 2*2 pc 28 tab อยากทราบว่าการใช้ cipro กับ warfarin เป้เน fatal drug interactions ด้วยหรือเปล่าครับ แต่แพทย์ยังยืนยันที่จะจ่าย cipro แต่ได้บอกไปแล้วครับ ส่วน omeplrazole กับ NSAIDs แพทย์ off ครับ แต่แพทย์บอกว่า cipro แค่ 7 วัน คงไม่เปนอะไร มันจิงหรอครับ??

ตั้งแต่ 0-8 สัปดาห์ = embryo 8-คลอด = fetus คลอดแล้ว = infant ไม่รู้ว่าที่ถามมาอยู่ในระยะใหน แต่ที่แน่ๆ เห็นมี warfarin อยู่ยังไงก็ใช้ไม่ได้ในคนท้องอยู่แล้ว ไม่ใช้หรอ ส่วน cipro ก็ลองเปิดตำราดูละกันครับหรือไม่ก็หาดูพวก paper ดู มาถามในนี้คนในนี้ก็ไปเปิดตำราไป serch ข้อมูลมาตอบกันทั้งนั้นแหละครับไม่ต่างกัน หาข้อมูลเองจะได้คล่องๆด้วยครับ แล้วที่หาไว้ก็เก็บใส่แฟ้มไว้ถ้ามีคนมาถามอีกก็หยิบมาดูได้ทันที ส่วนแนวทางการปฏิบัติ ก็ต้องดูเหตุผลของการใช้ cipro ด้วย ต้องพิจารณาเป็นเคสๆไป ซึ่งกรณีของคุณไม่รู้ว่ารายละเอียดเป็นอย่างไรคนไข้เป็นอะไรทำไมต้องเป็น cipro ก็ดูว่าติดเชื้ออะไร ที่ใหน ถ้าไม่ใช้ cipro เราจะใช้อะไรแทนที่มันปลอดภัย ก็เป็นหน้าที่เภสัชที่ต้องแนะนำแพทย์ไป

Warfarin - Omeprazole Sinificance : 4 Onset :delayed Severity : moderate Documentation : possible
Effects: The hypoprothrombinemic effects of WARFARIN may be increased. Mechanism: Stereoselective inhibition of the hepatic metabolism of the less potent R-WARFARIN enantiomer. Management: Monitor ANTICOAGULANT parameters when starting or stopping OMEPRAZOLE. Adjust the WARFARIN dose accordingly.
Discussion In a double-blind, randomized, crossover trial, the effects of coadministration of omeprazole 20 mg/day on the plasma concentration and anticoagulation action of warfarin were studied in 21 healthy men. Warfarin is a racemic mixture of 2 enantiomers, S- and R-warfarin, that differ in anticoagulant potency. The anticoagulant effect of S-warfarin is 3 to 6 times greater than that of R-warfarin. The anticoagulant effect and the plasma concentrations of the enantiomers were measured. To determine the dosage requirement for the vitamin K-dependent coagulation factors to decrease to within 10% to 20% of the normal range, warfarin alone was administered for 3 weeks prior to the crossover study. The final dose determined (2.5 to 8.125 mg/day) was maintained throughout the crossover phase. The addition of omeprazole to the dosage regimen had no effect on the mean concentration of the more active S-warfarin but did result in a small (12%) but statistically significant increase in the mean R-warfarin concentration (from 490 to 548 ng/mL). It was not necessary to adjust the warfarin dosage in any subject. The small effect of omeprazole on the anticoagulation activity of warfarin would not be expected to be clinically significant. In another double-blind, randomized, crossover study, 28 patients stabilized on warfarin were given omeprazole 20 mg or placebo for 3 weeks. Concentrations of R-warfarin increased 9.5% while S-warfarin concentrations were not affected. Coagulation times were not changed. A single case report indicates that certain individuals may exhibit clinically important increases in hypoprothrombinemic effects of warfarin after prolonged omeprazole therapy (20 mg/day for 2 weeks). Additional studies are needed to determine whether a larger, yet still therapeutic, dose of omeprazole would have a more pronounced effect on warfarin activity.

Effects: Increased ANTICOAGULANT activity and risk of bleeding. Mechanism: Gastric irritation and decreased platelet function contribute Management: Monitor patients closely and instruct them to report signs and symptoms of bleeding to their health care provider.
Discussion A large retrospective study of patients 65 yr of age and older receiving oral anticoagulants and nonaspirin NSAIDs reported a 13-fold increase in their risk of hemorrhagic peptic ulcers compared with patients not using NSAIDs. The risk of hemorrhagic peptic ulcer increased 4-fold during use of oral anticoagulants and NSAIDs compared with use of either agent alone. In a study of small- or large-bowel bleeding with NSAID use, NSAIDs increased the risk of bleeding about 2.5 times. In a 1-yr observational study, concurrent use of NSAIDs and coumarin anticoagulants greatly increased the risk of bleeding compared with coumarin alone. PTs have been altered during use of warfarin and indomethacin,ketoprofen,meclofenamate, nabumetone, fenoprofen (increased INR), naproxen, piroxicam,or sulindac. Others found that etodolac, tolmetin, ketoprofen, or indomethacin did not affect anticoagulant activity or decrease prothrombin activity (eg, naproxen).

Warfarin - Quinolone significant : 1 onset : dalayed severity : major documentation: probable
Effects: Increased ANTICOAGULANT effect of WARFARIN. Mechanism: Unknown. Management: If possible, choose a non?QUINOLONE antibiotic. Monitor ANTICOAGULANT activity more frequently when starting or stopping QUINOLONES and adjust the WARFARIN dose accordingly. Discussion PT prolongation was reported when warfarin and norfloxacin were coadministered. Increased PT and a lethal pontine hemorrhage occurred in 1 patient. No interaction was detected when 10 volunteers taking norfloxacin 400 mg twice daily for 9 days were given a midtrial dose of warfarin 30 mg. Case reports describe an interaction between ciprofloxacin and warfarin. The PT typically was prolonged after a few days of therapy in patients stabilized on warfarin. In 2 cases, marked PT and INR elevation with bleeding occurred. Another patient had a bleeding complication with a small prolongation. In 16 patients receiving ciprofloxacin and low-dose warfarin, a 22% PT prolongation occurred in 1 patient. In a double-blind study of 34 patients, ciprofloxacin 750 mg twice daily for 12 days increased levels of R-warfarin, while decreasing factors ?? and V??. Two cases of excessive anticoagulation were described when ofloxacin was administered with warfarin. In 1 case, the PT was 78 seconds with gross hematuria. Levofloxacin did not increase the PT in healthy subjects receiving warfarin. After adding levofloxacin to stable warfarin regimens, 11 cases of elevated INR occurred, including some with serious bleeding complications and death. At least 8 cases of elevated INR, including 1 with bleeding, were reported in patients on chronic warfarin therapy given moxifloxacin. In elderly patients receiving levofloxacin and warfarin, the rate of bleeding did not differ from that of control patients. Prolonged-release ciprofloxacin did not alter warfarin pharmacokinetics or pharmacodynamics.

ที่ยกมาเป็นข้อมูลจาก drug interaction facts เพื่อให้คนถามใช้ประกอบการตัดสินใจ ผมอาจจะไม่ตอบตรงๆ เพราะผมคิดว่้ามีประเด็นเยอะมากครับ ในการ consult เรื่องนี้ คุณน่าจะมีข้อมูลประกอบนะครับ ว่ายาแต่ละตัวมี indication อะไร และมีความจำเป็นมากน้อยขนาดไหนที่ต้องได้รับยานั้น มียาอื่นเป็นทางเลือกหรือไม่ ถ้ามีแล้วประสิทธิภาพมันแตกต่างกันมากน้อยขนาดไหน
เช่น เรื่องของ omeprazole จะเห็นว่าไม่ได้ห้ามใช้คู่กัน แต่ให้ติดตามดู INR
หากไปดูแนวทางการป้องกันการเกิด GI complications จาก NSAIDs จะเห็นว่ามีข้อบ่งใช้ของ PPIs เมื่อผู้ป่วยได้รับ anticoagulants ร่วมกับ NSAIDs ซึ่งน่าจะบอกเป็นนัยๆ ว่า ยาสามารถใช้ร่วมกับ warfarin ได้ แต่คงต้องติดตามที่ประสิทธิภาำพและความปลอดภัยของการใช้ยาร่วมกัน แล้ว H2RA ได้ไหม ? กรณี on NSAIDs อยู่ H2RA จัดว่าป้องกัน GI complications จาก NSAIDs ไ้ด้น้อยครับ โดย NSAIDs มักทำให้เกิด GU>DU แต่ H2RA ป้องกัน DU จาก NSAIDs ได้มากกว่า GU ตัวที่พอจะมี evidence ดีหน่้อยก็ famotidine ซึ่งต้องให้ยาในขนาดสูงมาก (40 mg bid) ประสิทธิภาพยังไม่เท่า PPIs เลยครับ ถ้าคนไข้เลี่ยง NSAIDs คู่กับ warfarin ไม่ได้ ก็คงต้อง on PPIs ร่วมด้วย

เรื่องของ NSAIDs อาจจะต้องดูว่ามาความจำเป็นมากหรือน้อยขนาดไหน หากเลี่ยงได้ อาจจะเลือก paracetamol (จริงๆ แล้วก็มี interaction กับ warfarin นะครับ) หรืออาจจะพยายามเลือกตัวที่มี GI SE ไม่แรงเช่น ibuprofen และพยายามให้ในขนาดต่ำๆ เท่าที่คุมอาการได้ และำพยายามให้ในระยะเวลาที่สั้นที่สุดเท่าที่จะทำได้

สำหรับ Ciprofloxacin ก็เช่้นกัน indication คืออะไร มียาทางเลือกอื่นไหม infection ยังไงมีแล้วก็คงต้องรักษาแน่นอน การจะบอกว่ามี DIs ให้ร่วมกันไม่ได้ คงจะต้องมีคำตอบให้แพทย์เช่นกัน ว่าแล้ัวจะใช้ยาอะไรรักษา infection คือต้องมีทางออกเช่นกัน (หากกลัวเรื่อง DIs อยากบอกว่า ABx ทุกตัวมี DI กับ warfarin หมดครับ มากบ้าง น้อยบ้าง หลักฐานดีบ้าง อ่อนบ้าง) ถ้าไม่มีทางเลือก ยังไงก็จำเป็นต้องใช้ ก็ต้องมีแนวทางการติดตามและป้องกันครับ อาจจะ couseling ผู้ป่วย และมีการติดตามอย่างใกล้ชิดเป็นต้น

ครับ ขอบคุณที่ชี้แนะครับ ผมเองเพิ่งจบอ่ะครับเลยว่าไปตามทษฎีที่เรียนมครับครับ ผมได้ consult แพทย์และคนไข้แล้วครับ แพทย์ไม่ทราบจึงได้สั่งออฟ pirox และ losec shift มาใช้ mydocalm ทนครับ ส่วน cipro แพทย์ใครับ ด้วยเหตุผลที่ว่าแค่ 7 วันไม่น่าเกิด ส่วนคนไข้ผม consult เรื่องการสังเกตอาการและการปฎิบัติตัวครับ ขอบคุณที่ชี้แนะผมอาจจะต้องหาข้อมูลให้มากกว่านี้

ไม่รู้ว่าน้อยใจผมหรือเปล่า ผมไม่ได้ว่าอะไรนะครับ แต่ผมแนะนำในกรณีว่า ถ้าต้องใช้ ควรทำอย่างไร ซึ่งก็คงเป็นการให้คำแนะนำผู้ป่วย และการติดตามผู้ป่วยครับ โดยแจ้งแพทย์ว่ามี interaction และความสำคัญของ DI นั้น ว่าจะทำให้ INR เพิ่มหรือลดมากน้อยขนาดไหน และถ้าเราจะไม่แนะนำให้ใช้ยานั้น เราควรมียาทางเลือกเสนอให้เค้าด้วยครับ เพราะหลายครั้งเค้าจะถามกลับแน่ๆ ว่าถ้าไม่ใช้ตัวนี้ แล้วมีตัวไหนใช้ได้ครับ

เรื่องทางเลือกผมเองได้เสนอให้แพทย์ไปแล้วครับ แต่ว่าตอนเรียนมาอ่ะ เรียนมาว่า NSAIDs ไม่ควรใช้อ้ะครับ เลยได้เสนอให้ใช้ mydocalm ไปแทนแล้วครับ ส่วน omeprazole ก็เรียนมาตอนติวอ่ะครับ เขาบอกให้เลี่ยงไปให้ Ranitidine แทน ก็เสนอไปแล้วเช่นกันครับ แล้วก็ cipro ก็บอกไปแล้วเหมือนกัน
